Housing transition services are designed to assist individuals in transitioning from their current living situation to a new residence in the community. This can be particularly important for individuals who are leaving institutional settings, such as hospitals, sober homes or long-term care facilities, or for those who are experiencing homelessness or unstable housing situations.

Once an individual has successfully transitioned to a new home in the community, the focus shifts to ensuring that they can maintain their housing and remain stable and secure in their new environment. This is where housing sustaining services come in.

Housing sustaining services are designed to provide ongoing support to individuals who are living in their own homes in the community. This can include a range of services, such as assistance with managing finances and paying bills, help with grocery shopping and meal planning, assistance with accessing medical care and other services, and support in maintaining social connections and participating in community activities.
